Aim

Evaluate the implementation of case management by a family medicine group (FMG) nurse and self-management support groups for FMG patients with complex needs.

Results

Case management by an FMG nurse improved the integration of care based on patient experience, reduced psychological distress and improved the sense of safety of patients and their families. Participants in self-management support groups reported an overall improvement in their self-management skills.
