New pan-Canadian project coming soon!

We are proud to announce that the project “Better understanding care transitions for adults with complex health and social care needs: an essential step towards integrated care” that we submitted to the Canadian Institutes of Health Research (CIHR) has been accepted!

This project aims to better understand care transitions between community, primary care and hospital for adults with complex health and social care needs.

To this end, our PriCARE team of patients, clinicians, decision-makers and interdisciplinary researchers from Quebec, New Brunswick and Newfoundland and Labrador will build on our successful teamwork over the past four years to conduct a study in six hospitals in these three provinces (two hospitals per province).
