In order to better understand and improve the care experiences of people with multiple health needs, our team would like to welcome people from diverse backgrounds. These people will join the team as patient partners, sharing their experiences and knowledge.

We are pleased to announce that we have been awarded a $2M CIHR grant in partnership with the Fonds de recherche en santé du Québec and Research New Brunswick as part of the Transforming Health with Integrated Care (THINC) initiative!

The project is entitled “PriCARE Integration – Case management in primary healthcare for people with complex healthcare needs to improve integrated care: a large-scale implementation study” and aims to scale up case management in primary healthcare for people with complex needs and generate new evidence to inform policies and healthcare system transformation towards more integrated care, to positively impact many Canadians.

Dr Catherine Hudon will lead the study with Maud-Christine Chouinard, Shelley Doucet and Helena Piccinini-Vallis, supported by knowledge users Kimberly Fairman, Philippe Lachance, Tara Sampalli, Greg Sargent, Joanna Zed and patient partners René Benoît, André Gaudreau, Donna Rubenstein, Linda Wilhelm and Audrey-Lise Rock-Hervieux.
